Endometriosis

Endometriosis Uncovered: Smarter Diagnosis and Holistic Healing for a Hidden Condition

By Mr Amr Moneib, Consultant Gynaecologist — TMC London

What Is Endometriosis?

Endometriosis is a condition where tissue similar to the lining of the womb (the endometrium) starts growing outside it — on the ovaries, fallopian tubes, bladder, bowel, or even the diaphragm.

These misplaced cells respond to monthly hormonal changes, causing inflammation, pain, and sometimes scar tissue that can pull organs together.

You’re not alone — it affects 1 in 10 women, yet many live undiagnosed for years.

Common symptoms include:
pelvic pain, painful periods, fatigue, bloating, pain during sex, and fertility difficulties.

Diagnosing Endometriosis: Why It’s Been Tricky

For years, diagnosis meant years of uncertainty. Many women were told period pain was “normal.”

Historically, laparoscopy (keyhole surgery) was the only way to confirm it. But things are changing.

Smarter Imaging: The New Era of Diagnosis

Recent advances in imaging are transforming how we detect endometriosis:

  • Specialised transvaginal ultrasound — when performed by trained experts, it can detect deep lesions involving the bowel, bladder, or ovaries with remarkable accuracy.

  • MRI for endometriosis mapping — high-resolution pelvic MRI can show adhesions, nodules, and involvement of nearby organs, helping surgeons plan tailored operations.

These innovations mean fewer diagnostic delays and more confidence in choosing the right treatment pathway.

Holistic Management: Because Endometriosis Is More Than Just Pain

Endometriosis doesn’t just affect the pelvis — it affects how women feel, function, and live.
That’s why management needs a holistic approach combining medical, surgical, and lifestyle strategies.

Hormonal Management

The goal is to suppress the hormonal triggers that cause the endometrial tissue to bleed:

  • Combined oral contraceptives (the pill or patch)

  • Progesterone-only pills, injections, or intrauterine systems (like Mirena)

  • GnRH analogues (temporary menopause effect)

  • Newer agents like Ryeqo, combining hormonal balance with fewer side effects

These treatments help many reduce pain and control symptoms.

Surgical Options

For others, especially those with severe pain or organ involvement:

  • Laparoscopic (keyhole) surgery allows precise removal (excision) of endometriosis deposits while preserving fertility.

  • Advanced centres use pre-operative imaging (MRI/ultrasound mapping) to plan safe, targeted operations — reducing recurrence and complications.

  • Some cases may involve multi-disciplinary teams with bowel or urology specialists.

Holistic and Lifestyle Care

  • Nutrition: Some dietary changes and trying exclusion diets can help reduce flare-ups.

  • Movement: Gentle exercise, yoga, and pelvic physiotherapy help reduce muscle tension.

  • Mental health: Chronic pain impacts mood; CBT and mindfulness improve resilience.

  • Community: Support groups and online communities remind you that you’re never alone on this journey.

Takeaway: Smarter Diagnosis, Kinder Care

Endometriosis deserves awareness, not silence.
With today’s smarter imaging, personalised hormonal therapies, and holistic care, women finally have real tools to reclaim control over their health.
